Definition
CERFA 14098*02 is the prior declaration form for a fireworks display, created by the order of 31 May 2010 under decree 2010-580. It is mandatory when the display uses category F4 fireworks or T2 articles, or when the total net explosive content exceeds 35 kg. It is sent to the mayor of the municipality and to the prefect of the department (the police prefect in Paris) at least one month before the firing date, by the organiser or by the pyrotechnician on their behalf. It contains the identity of the organiser and of the display supervisor with their certificate or approval number, the list of fireworks (category, calibre, quantity, net explosive content), the layout drawing with safety distances, the insurance certificate and, where relevant, the civil aviation opinion. The prefecture may ask for additional material or ban the display.
In practice, for the display company
A CERFA botched in June is a 14 July display left hanging.
